About the Job
- Processing Home Health Referrals and checking eligibilities
- Data entry in the EMR and Requesting insurance authorization if needed
- Coordinating a team of healthcare professionals to deliver home health services
- Maintaining a relationship with patients and their families, and keeping them informed about the patient’s condition and care
- Scheduling patient appointments and ensuring regular follow-ups
- Communicating with doctors, clinicians and other healthcare providers about patient progress or required assistance with the patient
- Ensuring compliance with healthcare laws, regulations, and standards
- Monitoring and ordering medical supplies and equipment as needed
- Resolving any issues or concerns related to patient care
- Pay is negotiable
