About the Job
Duties and Responsibilities:
- Processing Home Health Referrals and checking eligibilities.
- Data entry in the EMR and Requesting insurance authorization if needed.
- Coordinating a team of healthcare professionals to deliver home health services.
- Maintaining a relationship with patients and their families, and keeping them informed about the patient’s condition and care.
- Scheduling patient appointments and ensuring regular follow-ups.
- Communicating with doctors, clinicians and other healthcare providers about patient progress or required assistance with the patient.
- Ensuring compliance with healthcare laws, regulations, and standards.
- Monitoring and ordering medical supplies and equipment as needed.
- Resolving any issues or concerns related to patient care.
- Pay is negotiable.
